Union Sports Physicals Offered
Students must turn in a completed physical form dated after May 1 to the coach at the time of tryouts for the 2010-2011 school year. There will be no exceptions. Tryout schedules will be posted when applicable.
Physicals performed by an M.D. or D.O. are recommended.
Physicals are mandatory. In order to tryout or participate in any athletic activity for the 2010-2011 school year, the athlete must turn a current physical into the coach at the time of tryouts. Please do not give the original of this physical to the coach; turn in a copy. Only physicals dated after May 1, 2010, will be accepted for tryouts and will be valid through the end of the 2010-2011 school year.
The annual Sports Physicals Night will be Monday, May 3, from 6 – 8:45 p.m. at the UMAC. The cost of this physical is $15.
Physicals performed by your family physician must be completed on the OSSAA Physical Form, which is available at the front office at each school, the Athletic Department at the UMAC (2nd Floor), or may be downloaded from the Union website.
